other agencies, as well as self-referrals.

Referral Process:
Children and Families may receive services from C.I.C. at
the request of other community agencies or organizations.
After receiving the request and determining the eligibility
of your child, there is a 4-step process that will assist in
developing individualized services for your child and
family. In addition, this process also ensures that we are
meeting all Medicaid guidelines for establishing new
clients.
Your Responsibility: Every attempt will be made to coordinate
the delivery of services to accommodate you and your child’s
availability. It is our hope that you will work closely
with your intervention team in the following areas:
Making and keeping
scheduled appointments
Ensuring that
child/children are available for scheduled in-home
appointments
Participating in the
development of your child and families’ treatment/
service plan
Participating in
interventions by providing input and feedback regarding
the child’s behavior, response to interventions,
response to medication, and overall services to your
child/children and family
It is our goal to provide the highest
quality services to your child and family with a focus on
building youth and family strengths, and nurturing a healthy
home and community environment.
